$195 Terms and Conditions
This program applies exclusively to metal braces and is not available to patients with an active payment plan. Materials upgrade is available for different braces type for additional cost. The practice reserves the right to charge for additional services not covered by the initial agreement. By enrolling, patients agree to adhere to all payment terms, eligibility criteria, and conditions. Automatic payments are required. Should the responsible party choose not to be added to automated payment schedule, payment in full will be required. All participants must provide a valid Social Security Number. Non-compliance may result in termination of treatment.
For early interceptive treatment or patients that can provide either a state denial status or letter for comprehensive treatment, an initial payment of $195.00 is required on the day braces are placed, with the remaining balance paid through recurring monthly payments of $195.00 via automatic deductions until the treatment cost is fully remitted.
Self-pay patients can take advantage of this program but are required to make a $500 down payment at the start of treatment, with the remaining balance paid through recurring monthly payments of $195.00 via automatic deductions until the treatment cost is fully remitted.
This program cannot be combined with insurance discounts; however, patients with PPO insurance may be eligible for $195 monthly payments, details will be determined during the consultation.
