Hoosier Healthwise currently works through managed-care plans including Anthem, CareSource and Managed Health Services. Program participation can change, so bring the patient’s current card rather than relying on an old provider directory or previous benefit check.

Plan acceptance does not guarantee approval for braces. The patient must have active eligibility and satisfy Indiana’s clinical and authorization requirements.

What happens at the consultation?

• Whether orthodontic treatment is needed
• The nature and severity of the condition
• Whether treatment should begin now or be monitored
• Whether the case may meet Indiana Medicaid requirements
• Which records and administrative steps are needed

The standard consultation has no consultation charge to the patient. Applicable consultation or diagnostic services may be billed to insurance.

Patients should continue routine dental care. Active cavities, gum inflammation or other oral-health problems may need treatment before braces can be placed.

Will Indiana Medicaid cover the braces?

Indiana Medicaid may cover medically necessary orthodontic treatment for eligible members age 20 or younger.

Qualifying cases can involve severe bite problems, trauma, impacted teeth or craniofacial conditions. All orthodontic services require prior authorization.
