Hoosier Plans for Indiana Orthodontic Coverage
In the state of Indiana Medicaid consists of several different programs, each serving a different population. There are multiple health care programs including Healthy Indiana Plan, Hoosier Care Connect, Hoosier Healthwise, Hoosier Rx, Medicare Savings Program, Pharmacy Benefits, Traditional Medicaid and Indiana Medicaid Covered Services.
Hoosier Healthwise
Hoosier Healthwise (HHW) is Indiana’s health care program for children, pregnant women, and families with low income. Children up to age 19 may be eligible for coverage based on family income. Hoosier Healthwise covers various types of medical care including prescription medicine, doctor visits, mental health care, dental care, hospitalizations, surgeries, and family planning at little or no-cost to the member or the member’s family.
Hoosier Indiana Orthodontic Provider Information
The HHW program is operated within the managed care delivery system. Contracted managed care entities arrange, administer, and pay for the delivery of healthcare services to members enrolled in their health plan. All medical care services, except Medicaid Rehabilitation Option services, are provided through the managed care entities, including dental and pharmacy services.
There four MCEs contracted with the state of Indiana to serve the Hoosier Healthwise population. These MCEs include Anthem, Care Source Indiana, Managed Health Services (MHS), MDwise. Only two out of four covers orthodontic treatment. And these two are Anthem and MDwise.
MDWise
Under Hoosier Healthwise, DentaQuest provides your dental services in Indiana State. Dental care is very important for your health and well-being. You can choose a dentist to have regular check-ups every six months at your dentist’s office. The dental services that are covered under Hoosier Healthwise have three different packages.
Package A Adult
Consist of One exam and cleaning per year, Bite-wing x-rays once every 12 months, one complete set of x-rays every 3 years, Restorations such as fillings and stainless steel crowns, full dentures, immediate dentures, and repairs to partials and dentures Extractions, Periodontal care includes deep cleanings and surgical treatment for gum disease Partials.
Package A Child
Includes Two exams and cleanings per year, Fluoride varnish, Sealants, Bite-wing x-rays once every 12 months, one complete set of x-rays every 3 years, Endodontic procedures such as root canals, Periodontal care includes deep cleanings and surgical treatment for gum disease, Restorations such as fillings and stainless steel crowns, Partials, full dentures, and repairs to partials and dentures, Extractions, Sedation and nitrous oxide if medically necessary.
Package C CHIP

Dental Services by Anthem under Hoosier Healthwise consists of Exams, Cleanings, X-rays, Fillings, Fluoride treatment (age 20 and under), Crowns, Extractions.

One of the best ways to apply is through the Division of Family Resources (DFR) and other authorized enrollment centers. To find the DFR office or enrollment center close to where you live or please call the Hoosier Healthwise Helpline.
You may also apply online, by phone or through the Federal Health Insurance Marketplace. To apply for Hoosier Healthwise You may need Proof of income, Proof of citizenship or immigration status and proof of identity as well.
